Recently the Board has received several inquiries regarding nurse licensees who were identified by their employer as having a different name in the employer’s database than what appears on the Board’s website.
All West Virginia Board of Registered Nurses initial and renewal applications require the applicant to enter their “Full Legal Name.” For obvious reasons, it is imperative that the Board has accurate names for its licensees.
In the event that you have legally changed your name since your initial or renewal application, under 19CSR3.7 Change of Name and/or Address: 19CSR3.7.1: “If a licensee legally changes his or her name through marriage, divorce court order or other means, he or she shall send this information to the office of the board. The information shall include both the full prior name and the new name, in a properly executed affidavit or a certified copy of the marriage certificate or divorce decree. The licensee shall submit an application and these documents to the nurse portal.
If you have simply entered a name that is not your legal name by accident, please log into your nurse portal and fill out the Name Correction Application. It is available in the "Other Applications" section, below the licensing applications. You will need to attach a copy of your identification (drivers license or birth certificate).
Please note that pursuant to 19CSR3.12.1.jj. “Provided false information on any application, or any other document submitted to the board for the purpose of licensure, advance practice recognition, or prescriptive authority” can be a basis for disciplinary action.
