Disciplinary Action Summaries & Board Orders

Official public disclosures of formal Board actions, consent agreements, reprimands, license suspensions, and reinstatements issued by the West Virginia Board of Examiners for Registered Professional Nurses.

⚠️ Check Real-Time License Status

Discipline reports are published and updated on a monthly basis. Prior to taking any employment, privilege, or credentialing action based on these lists, please verify a nurse's real-time license status through our Online Verification Portal or via the NURSYS National Database.

Types of Board Actions

Select a disciplinary category below to view statutory definitions and practice limitations under West Virginia law:

Consent Agreement (Probation)

A Consent Agreement is a formal settlement between the Board and the licensee resulting from an informal disposition of a complaint. Agreements requiring probation generally impose practice restrictions.

Note on Probationary Period: Time is counted toward the required probationary period only while the individual is actively working as a registered professional nurse. If a nurse is not actively practicing, that time does not count toward completing probation.

Reprimand

A Reprimand is the least restrictive formal disciplinary action taken by the Board. A licensee is permitted to continue practicing nursing without restrictions while a reprimand is part of their official record.

Suspension

A Suspension halts a licensee's legal authority to practice nursing, typically resulting from disciplinary action or a violation of a Board order/contract. A nurse cannot practice nursing in West Virginia while their license is suspended.

Summary Suspension

A Summary Suspension is an immediate action taken when the Board determines a licensee poses an immediate threat to public health and safety. The individual must immediately cease practicing and cannot represent themselves as a registered nurse.

Reinstatement

A Reinstatement occurs after a licensee successfully completes all required disciplinary terms. Reinstatement may restore a license to unencumbered status or transition a suspended license into a probationary period.
