All complaints must be submitted electronically through the West Virginia Board of Registered Nurses official portal. Complete and submit your formal complaint online to initiate a review.

Understanding the Complaint Process
Complaints may be submitted by anyone who has concerns regarding a nurse's practice or professional conduct. Common sources of complaints include:
- Patients & Family Members: Reporting care concerns, communication issues, or professional boundary violations.
- Employers & Healthcare Facilities: Reporting mandatory incidents, standard-of-care failures, patient abandonment, or medication diversion.
- Co-Workers & Healthcare Providers: Reporting suspected impairment, unethical conduct, or unsafe practice.
- Law Enforcement & Government Agencies: Reporting legal convictions or official regulatory enforcement findings.
Important Information for Filers
Mandatory Reporting Obligation: Employers and licensed nurses have a statutory obligation under West Virginia law to report known violations of the Nurse Practice Act, including unsafe practice, substance impairment, gross negligence, or criminal behavior.
Substance Use & Impairment (WV Restore)
If a concern involves substance use disorder, mental health challenges, or physical conditions impacting safe practice, nurses may be eligible for confidential monitoring and recovery support through WV Restore.
- Voluntary self-referrals made prior to a formal board complaint remain confidential and non-disciplinary.
